If you have an error or omission on your DD214 or other Army
records, a request can be sent to the Army Review Board if you would like to get
your Army records reviewed, updated or corrected. Form
DD149 "Correction of Military Records" provides instructions, and a mailing
address on how/where to submit that request. |

Request Missing or Lost
Military Awards and Medals |
Use this link to request missing or lost Military Awards and
Medals. They only only authorize medals that appear on your DD214. If you would
like to get your records reviewed, updated or corrected a request can be sent to
the Army Review Board. Use the DD149 form shown above which provides
instructions, and a mailing address on how/where to submit that request. |
VA Form 21-534 |
Application for dependency and indemnity compensation, death
pension and accrued benefits by a surviving spouse or child (including Death
Compensation if applicable).
You must include a death certificate which
states the cause of death (i.e. cancer of bladder, liver, colon, prostate etc.).
Even if
it's not on the list of presumptive illnesses, fill it out anyhow and include
any other medical paperwork that concerned the veteran before he or she died.
If you are unsure (or unable) how to answer any particular question, fill out
the form as best you can. Also include
a copy of the DD-214. While the forms tell
you to mail
the completed form to your nearest VA Regional Office, we have been advised that
the best thing to do is get it to the Vietnam Veterans of America (VVA) rep in your
state who will take this form and represent the case of each individual with the
Veterans Administration reps. The VVA seems to have the best reps, compared to
the other agencies. Not to say a National Service Officer of the Disabled
American Veterans (DAV) or other Veterans organization wouldn't do as good a
job. Unfortunately, mailing anything yourself to VA facility often results in it
being lost or ignored.
