We all know that Vietnam Veterans suffer a high number of health problems. As a potential benefit to all of us, I would like you to complete this health survey.  All responses will be kept confidential.  All of your answers will be consolidated into a spreadsheet and this information will only be used as a resource for when one of us want to know the frequency of various diseases/illnesses among our group.

Open the file, do a File "Save As" to a location on your computer, complete the survey using MS Word, and then email the completed survey to artillery_83rd@yahoo.com
